Transition of Care from Adolescent to Adult Providers among High Primary Care Utilizing Patients

Purpose: A smooth transition from pediatric/adolescent to adult healthcare is important for continuity of care, which has been associated with improved health outcomes, including lower frequency of hospital admissions and emergency room visits and increased use of preventive services. Transition has been prioritized by the American Academy of Pediatrics, American Academy of Family Physicians, and American College of Physicians, but their recommendations have been based on expert consensus due to sparse data obtained mostly in sub-specialty populations.
